Custom fields refactor to better support date syncing

Our custom fields - Program and Program Increment - can be used for reporting and integration with BI tools like eazyBI.

There are instances where custom fields are not added to issues scheduled into a program that uses date syncing.

We are planning a refactor to ensure custom fields are added to all issues in your program.
